This California Durable Power of Attorney for Property free PDF download allows you to appoint an agent to manage your property in the event that you become incapable of managing it yourself.

For this legal form to be valid, your signature must be notarized.

This form will come into effect only if two licensed doctors state in writing that you are unable to provide for your financial needs. The doctors must be unrelated to you and not related to any nominated agent.
