
This free download, the Guardian of the Estate and Person form for California, allows you to nominate the person who you want to care for your minor children in the event that both parents pass away.
You can nominate one person to manage the child’s personal affairs such as education, medical decisions, curfew, etc. (the “guardian of the person”), while nominating someone else to manage the property that you leave for that child (the “guardian of the estate”). Or you can also nominate one person to perform both functions.
Please be sure that each parent completes a nomination form for each minor child.

This form was created by the California legislature. It is intended to be self-explanatory so that non-lawyers can complete it without an attorney. The form is consistent with Section 1204 and Sections 1500-1502 of the California Probate Code.
